What Is Support Coordination?

Who Is Eligible for Support Coordination?

Not every participant automatically receives Support Coordination. The NDIS may include it in your plan if you:

  • Have limited informal supports (e.g. family or friends)
  • Are new to the NDIS and need help understanding how it works
  • Have complex needs or multiple providers
  • Are in a time of transition (e.g. leaving school, moving house, hospital discharge)

How to Request Support Coordination in Your Plan

To get Support Coordination included, follow these steps:

  1. Speak with your LAC or planner – Let them know you need help managing your plan and explain why Support Coordination would help.
  2. Provide evidence – Support letters from a doctor, therapist, or social worker can help show why Support Coordination is reasonable and necessary for your situation.
  3. Prepare for your planning meeting – Write down your goals and challenges and bring any documents to support your request.

Can I Ask for It During a Plan Review?

Yes. If you didn’t receive Support Coordination in your current plan, you can request it at your next plan reassessment. Make sure to highlight any challenges you’ve faced without it and how a Support Coordinator would help improve your outcomes.

What Happens Once It’s Approved?

If the NDIS includes Support Coordination in your plan, you’ll have a set budget allocated under the Capacity Building – Support Coordination category. You can then choose a registered provider—like Angels on Wheels—to deliver the service.
